iHubNet iForm Center - Private Nameserver Setup Form

IMPORTANT

Please fill out each field for the reseller account for which you would like to set up a private nameserver.
Also, please set up the two hostnames you would like to use for your nameservers at your Domain Name Registrar using two of your dedicated IP addresses.
For "Email Address(ID) on My.iHubNet:", please enter the email address that is used as your My.iHubNet and iHubNet Helpdesk login. If you do not use the same email address, it will be rejected by our system and we will not receive your request.
